You go up in grade (level) by earning skill points, you go up in rank by spending skill points.
To be promoted from Ensign to Lieutenant, you need to spend 1500 skill points in total (exactly the amount you earn from the introductory mission sequence, ending when you speak to the Admiral at Earth Spacedock).
To be promoted to Lt Commander, you need to spend 6700 skills in total.

I think what you are looking for is this:
You can be a LCDR Lvl 10, but you won't actually be able to promote to CDR until you completely finish off Lvl 10. The screen will actually say LCDR Lvl 11, until you spend the skill points you earned on the mission you just did to cross over to Lvl 11. Once you spend the skill points, bada bing bada boom all of a sudden the bar at the top now says CDR. You can then go to the Sol Starbase and have a little promotion if you want.

Clarification - I do not mean Lvl 10 from the beginning of the game, rather 10th level/rank/grade of Commander. Each rank has 10 levels/grades/ranks, whatever you want to call them. This is what I was referring to.
