It seems like its random, and NO Im not procing my high yield at the time...any ideas?
No ideas, but I've definitely seen it two. It happened a lot in a mission early on where I was firing at unmoving, unshielded Orion space docks -- I think it was going off pretty much every other time I fired a Photon at a space dock in that mission. I haven't seen it much since, but I still do occasionally see it.

It's a graphical bug. You'll notice that only one of those torpedos actually does the damage. It happens a lot if you fire right as the game is trying to determine if you are within firing arc with no obstructions. This is why you'll notice it a lot if you're spamming the fire torpedo key while turning into it's arc.
