Help: Teamed up and missions don't complet...

Has anyone had issues where you are on a closed team with a friend, and you have the same missions in your log, but for one of you, the mission does not complete?

We did the P'Jem mission and, I think, Hide and Seek. Both times we had to repeat in order for my friend to get the quest rewards. Is it something that we're doing wrong or is it a bug.

    I was in a party of four for City on the Edge of Never and I had to do from the part where you first meet the Enterprise to the end four time to get a mission reward.
