Has anyone elses shuttle vanished from their bars? mine has gone and when i visit the c-store its not there. has it been removed for the launch? apologies if there is another thread on this as i cant find one.

Yes, happened to me and husband also. We even bought new cups/codes and tried inputting them, but it says this type of reward has already been claimed.
Inside STO, just prior to the Shuttle disappearing from C-Store, it gave a message indicating that this reward could only be claimed by one character.
1. Can only one code be used per account?
2. Is this code then useable by all characters on the account?
3. How do we access it after we used it on our accounts during Beta?
The hotbar slots have tendency to shuffle from time to time. Though I did have a quest reward shield disappear from my ship last night, so I know it happens.

Yes, you could get your shuttle this weekend. I logged in and claimed mine on Friday, Jan 29th.
We applied our code during Beta and the Shuttles worked fine.
Now that the accounts have been reset for Head Start, the Shuttle Option is no longer present. It is emply as if we never claimed one.
When we use a brand new code, the website states we have already claimed this reward.
So our questions still remain.
Were they just useable during Beta and now not during Head Start?
On Saturday, Jan. 30, the C-Store showed the Shuttle Option present but greyed out and gave an error stating that the Reward can only be claimed once per character?
If it is once per character as the ingame message indicated, why can we input a new code now and use it on this new Head Start character?

My guess is that everything got reset with the headstart, but somehow you were only allowed one shuttle. That unfortunately did not reset on your account. It seems that everyone who entered it after headstart is fine.
Mine's gone, too. I submitted a ticket to a GM and got a reply that I need to contact billing support. Sigh, you're probably in the same building, so why don't you contact billing?! Oh well, I'll just wait till tomorrow and if it doesn't appear, I'll wait on hold for 10 days to contact billing.

I had read in a forum post prior to that that the shuttle was not claimable for more that one character on an account. That is obviously a bug because I remember reading the item in the Cstore and it said, "This item can only be claimed once per character." I also remember reading that some people were unable to claim the shuttle if they were using the same character name in live as they did in beta, as the Cstore read them as having already claimed it.
I assume they removed it for now so that they could fix these issues.
