I think I've found a reproducible bug by which you get no item reward for a quest. Reproduce as follows:

-Finish a quest that has an item as part of it's reward.
-Hail Starfleet, Talk to the appropriate contact for the quest.
-Open the final dialogue screen for the quest (ie: the one that shows what the rewards are)
-Close this screen without completing the quest.
-Now, Hail Starfleet again.
-Again, Open the final dialogue screen for the quest.
-Select your reward, and complete the quest.

You will receive the skill points, merits and credits for the quest, but no item.

Ya know what, I might have done that for my missing bridge officers, or similar. I brought up the assignments tab so I could look to she what my current abilities are. I am not at my computer at the moment, but as soon as I am able I'll check to see if I can replicate what you found. Nice one :)

Something similar happened to me earlier in the week. I was supposed to get a BO in one of the early missions. I never got him. I didn't bug it because I did see that I got the merits, xps, and credits. I guess I thought that I misread the mission reward (the BO)...it makes sense - I didn't know how I did it, but it is possible I did what you did.

Too bad they won't include the mission rewards in the journal entries - that would super helpful.

Yeah, I became more sure of this when I went and looked through ALL my bridge officers (to see how the blue ones were different) before trying to accept any of them.

I accepted my first one: nothing appears
    then my second: nothing
    3-6: whole lot of nothing
