Definitely a driver or hardware issues here.

What I would do is completely wipe the driver and install the latest...
    • Run the NVIDIA uninstaller (from Add/Remove Programs in XP or just Programs in Vista/7).
    • Download the Guru3D DriverSweeper (free, trusted software).
    • Download the latest NVIDIA driver package that includes support for your card.
    • Reboot your PC in Safe Mode (keep pressing F8 after the BIOS/Firmware screens and then select Safe Mode... F5 does the same thing).
    • Run Driver Sweeper and select all NVIDIA display drivers for removal if any exist. Analyze and clean.
    • Reboot normally.
    • Install the new NVIDIA driver and reboot when prompted.

I've never had a problem with uninstalling NVIDIA drivers, but I hear others have and this might help.

Next, I'd reinstall DX9 on your PC. You can download this manually from Microsoft. Reboot after doing this.

If you're still having problems, then I'd take a guess that you have a defective video card.
