Traits, items, and abilities become unslotted or changed

So over the past month and a half to two months I've noticed that certain abilities, items or traits of mine will become unslotted or changed, at first I ignored it, but now it's starting to become annoying, I was doing a story mission where I had to go defend earth spacedock last week and the battle started and for some reason and at first I couldn't figure it out but my ship wasn't moving any faster then a crawl, and when I went to check, my engines, warp drive and half my weapons had be unequipped and put back into my inventory, and to make matters worse, the addition of all items overloaded my inventory so I had to discard things (items there were going to be discarded anyhow so no big deal with that) but it took me a couple minutes in the heat of battle to do this just so my equipment would show back up and I could re-equip them, but not cool. On more then one occasion after I complete a mission where the story mode has me using a different vessel or when I change my vessel due to whatever situation I am getting ready to go into, when I switch back I fing that most of all of my traits have been wiped clean and I have to go and re-select all of them I don't always remember what I had equipped. And last there are certain patrols I will be doing to get my dilithium and marks and my equipment is not strong enough yet, I know that so after a few times respawning the game respawns me right back where I blew up, meaning right in the middle of an enemy cluster where I am immediately fired up on, and destroyed again within 10 sec, a lot of times with no time to get my engines going to escape, leaving me to whittle away at them slowly until I have destroyed enough just so I can put distance between me and them, and each of those times as I can't abort a patrol during combat I don't have enough time to abort before I am fired upon, I get destroyed and right back to to same spot I was destroyed, right in the middle of all the enemy ships, I try to run, I'm destroyed before I can get out of range of their weapons, and right back to where I was, right in the middle of the cluster, leaving re-loading the game as my only way to exit that patrol, I can't imagine this can be anything other then a bug, because who designs a game where when you respawn it brings you back right in someone's crosshairs
