TFOs getting stuck

I don’t know if you’re aware as I haven’t seen any acknowledgement about it but, the TFOs keep getting stuck. I’ve had it on several occasions where on the undine assault advanced the command ship will not scan the rift to advance the mission.
I have also noticed that on Swarm TFO after the mine satellites are repaired the mission will not progress. It’s says destroy remaining hurt, yet they have been destroyed?!
