Did not receive 'Featured Episode Weekly Reward Box'

Are there any known issues with the featured episode rewards? I just completed the mission and did not get a Featured Episode Weekly Reward Box. This is the first time I am running the mission on my account since week 3 started and I did get all the other rewards, just not the box.

    Ran it again now on a different character (same account) and got the reward this time.
