ps4 help

hi, i started playing the agents of yesterday on PS$, and i have a few questions. i tried googling the answers, but i got nowhere.
1. on the event that gives the naussican gear as rewards, the item description mentions a torpedo launcer, ive done that mission over 4 times and have yet to see it. where does it come from?
2. i'm currently on the mission "city on the edge of never" and there's an objective to contact a temporal agent. how do i do that?
3. my grenade ability stopped working properly. now, if i set it to the L1 button, all i get is an inoperative black square, and after a few seconds, it changes to another ability. how do i fix it so i can use grenades again?

The rewards for that mission are staggered to release once a week over four weeks. The first week was the Beam Array, the second week was the console, and the third week (starting yesterday) was the Torpedo Launcher.

If you complete the mission again now, you'll have the option to take the Torpedo Launcher.

If you complete the mission in a week's time, you'll be able to take the fourth reward whatever that may be.

You have to find him and talk to him.

If its the mission I'm thinking of, at one point you're on the bridge of a ship. Look behind a door at the back of the bridge to find him.
