For me one of them is the dream pvp revamp. On one hand, next to the foundry pvp could become the biggest source of player made content and I also love a good scrap.
On the other hand, Feds and the KDF are filled to the brim with people I really don't want to fight along side so any real universe changing pvp will become annoying to me really fast.
The easiest fix would be a free for all mode but I'm sure most pvpers would not that handle that and the queues would die.
The other thing is something LOTRO's music system. The part of me that is for it thinks about turning your boffs into your band and klingon war drums during duels.
The other part of me thinks about what new level of trolling will be done with it.
Not sure if there is a easy fix for that. So what do you kind of want?

Non-combat missions and missions that have multiple methods of completing them. You could use gunboat diplomacy or you could use diplomacy to resolve matters like a civilized individual.

On topic, outfits. On one hand I think it would be fun if they went hogwild and just starting starting what they wanted into the c-store, lore friendliness be damm.
On other hand, as you said. STO is pretty much hanging on by a thread to "true" Trek and I'm pretty sure someone rocking a "CHiPs" styled bike cop outfit would cut that thread.
On the one hand, it would be great to see them as they are/were the fourth main power introduced to us in the show, and they were a driving force behind DS9's success I think. The characters of Dukat, Damar and Garak added greatly to the diversity of the show.
On the other hand, I'm (to be blunt) disgusted with how the Republic has been hung out to dry, and I would hate for a Cardassian faction to suffer the same fate. I did, and I always will support full factions over the mini-faction counterpart.

If I understand the OP right, I would want ship interiors reworked, player housing or linked to foundry.
Would the work be worth what they could sell for, you'd have to sell TRIBBLE in the cstore for interiors to make the work worth it. Hell you'd have to sell interiors to make it worth it, and past the fact that maybe oddy bridges for any ship might sell (and the current VOY interiors should be a no-brainer), I don't see it being worth it.
Plus what am I going to do, after I make it all "pretty"?
