Platform Elitist Accolade

I'm trying to finish the last missing Accolade from Dust to Dust. In 90% of the Times I die and have to start again. I made it through the Mission without dying twice. Both times I did not get the accolade. Is there any hidden condition or trick?

    Are you selecting the Very Hard option when Harry prompts you? I'm pretty sure that the difficulty selection is the only qualifier, as I've gotten it despite taking a plunge into the abyss during the run.

    I did the hole Mission without dying and the ramp part on very hard, but no accolade. It seems to be buggy.

    PS: My toon is lvl 17.
