Ok, ever since I figured out how to make my headphone mic work (had to really crank some driver settings in windows), I've been dying to hear some of the drivel in Zone chat and to have voice available for pugs, etc.
My fleet uses Vent, and that worked fine, but everytime I join an in-game channel I first get the message "You have joined a voice chat channel" then "Failed to join a voice chat channel".
Where should I look to figure this out. My in-game audio options seem to look good at a first glance.

There is no voice in zone chat. Only Fleet has its own channel; or groups you're teamed with have a channel created on the fly, of which basically nobody uses the voice chat in (seriously, in more than a year of random stf pugging I've only heard other players on the in-game voice chat maybe 4 times, and that was only because they didn't realize they had open mic turned on [so they weren't chatting, it was just the sound of keyboard mashing and game sounds from their speakers]).
Vivox was basically stillborn due to shoddy implementation (including the aforementioned lack of voice in map zones, lack of visual chat cues, lack of chat UI, etc.) that Cryptic has no interest in fixing. The only people I know that actually use it are my fleet because they don't want to roll a Vent/TS/Mumble server despite my hounding.
As for this bug:
"You have joined a voice chat channel" then "Failed to join a voice chat channel".
I've noticed that seems to happen mainly when you switch from a character on one faction to one on the other, and I know of no way to fix it other than to restart the game or just disable voice chat entirely for the remainder of your play session. I think it's erroneously trying to connect to the fleet voice channel from your opposite faction's fleet, which of course isn't possible since it's the opposite faction. It just gets stuck in that loop. Or it might be trying to join a team channel for a pug that doesn't exist anymore since you completed the stf etc.
Nitpicking is a time-honored tradition of science fiction. Asking your readers not to worry about the "little things" is like asking a dog not to sniff at people's crotches. If there's something that appears to violate natural laws, then you can expect someone's going to point it out. That's just the way things are.
