I had a quick look but didn't see another thread about these. I just picked up a pair of Tactical BOffs for my new Tac character but when I went to the bridge I saw that the male had been playing with a Disruptor and managed to hit himself with a negligent discharge. It seems his head came off a little worse for wear...
I believe they were green quality BOffs from the Embassy, after selecting them the portrait for this guy came up as normal but as soon as he was accepted his head disappeared both on the bridge and when viewing the paper doll in the Character/ Officer status screen.
