Might be helpful if you gave us some specifics. You mention unplayable but that doesn;t give us much to work on. Are you getting some sort of error? did the graphics change?
Could your internet connection be especially laggy?
We can only guess at the problem, until you provide more details.

Game's jerky and laggy 50-70% of the time for me, and I still kick TRIBBLE. One quickly gets used to it. I could lower the graphics and stuff, but then I'd miss out on all the artwork that Cryptic presumably spends a lot of time working on :S
My PvP toon is Krov, of The House of Snoo. Beware of my Hegh'ta of doom.
I'd been having problems ever since the last patch with lag, rubber banding and server disconnects. Ran thru everything on my end I could find including: reinstall NIC drivers, reboot network, cleared all system caches, reinstalled Direct X 9 and graphics drivers. However the issues appeared to be connection related rather than processing glitches of some sort. I did all sorts of tests on my connection, no special issues... got good thruput for the most part. Finally got 'round to my AV/Firewall. I use Avast! Once I started thinking in that direction I remembered they came out with an upgrade just about the same time as the last STO patch. Turned off Avast! and logged in to STO.... TAH DAH! Looks like that was the culprit. I still have the lag issues everyone seems to get that you learn to live with.
If any of you experiencing similar issues are running Avast! try turning it off. You can also plop down the money for the paid version upgrade which is fully configurable and will allow you to set up DMZ's thru the firewall... OR just make sure everything else on your system that might connect to the internet is turned off then disable Avast! (there is an option to disable it until the next reboot). Log in, play the game, log out, reboot... yer good to go.
If Avast! isn't your issue and you run Windows XP make sure the Windows Firewall hasn't gotten turned on. That happens sometimes. I've seen several times where the Firewall was inadvertently turned on. That Windows XP Firewall can be a real thruput killer... :eek:

Hmm...try resetting your graphics back to default, if you can. I tweaked my graphics to get rid of some blindingly bright objects on space maps, afterwards I had major issues on mission maps with objects not loading, floors, walls, objects, you name it. I once went into a space mission where I could only see a solid wall of black and my own HUD.
The only issue I've had since then is rubber-banding all over the place, hopefully from server side lag.
