It's nice to know some things never change about online games; and if people can be Loot Ninjas, they will be, at every chance they get. STO was always very good at preventing this, but sadly the most recent patch has opened the field wide for Loot Ninjas everywhere to flock to the STFs. It's especially sad since these instances are now far more playable, and more fun, than they have ever been before. But since every instance is going to get Ninja'ed by some jerk who unteams just as the optional gets completed, they're just not worth it.
Hate to say this...as I know I will get flamed... but thats why you join up with a good fleet. Even though Im in the 7th Strike Fleet I frequently team up with a couple other fleets to run STFs, and one thing we all enforce is NO LOOT NINJAs...
It's not loot ninja.. it's a problem with the Queue disbanding the team before you can get to the loot. It should be fixed with the Tribble patch next week.
In pve queued maps the teaming is controlled by the server.. it just has a bug at the moment.

Well, I see five people in team for most of the instance. Then we get the optional completion, and I notice now there are somehow only four people in team. And there is one guy circling the team loot floater. Then it disappears. Then he disappears.
I don't know what they call that where you come from, but I call that: Loot. Ninja. I get that it has something to do with team disbanding, but people do seem to have figured out how to exploit it. And I'm not seeing the whole team dissolve; just one person drop out, grab the loot, and leave.

You can't ninja loot in a pve queue map. You are not able to drop the team (unless you leave the map) until the mission is complete. Right now there is a problem where the queue is dropping the team at the end of the mission instead of when the players leave the map, and only one person can access the bag at that point.

The closest player does not always get the loot. The reference you gave could have been a player getting kicked. The optional also failed if the 5th person joined to late. Give next weeks patch a chance to fix it.
If they're circling the loot bag and it disappears, that sure looks like they got it. Especially when they disappear immediately after that. Maybe they never got teamed; or maybe the got dropped somehow; but however they got out of the team, they did apparently have access to the bag, because nobody else was close to it when the gold floater for the bag disappeared.
Only experienced this on Infected, since the Optional completes at the end. But on that map, unless you for a team (which you can do after you auto disband) Nobody can click the take items dialog. It just pops up again and again. And the funny thing is, you have to reteam all 5 people, or the need/greed timer refuses to run out. So at the end of Infected, nobody leave, reteam and grab the optional. Cure is better this way, since the reward comes before the carrier decloaks.
