This started after the last major patch that removed the Reman BO from the mission rewards for Cutting the Cord. This only happens on ONE of my characters that I have, doesn't affect the others at all. On my Tellarite tac officer, during ground missions, I getting a frequently played sound effect that is very very loud. It seems to play when he takes damage, of any sort. Weapons fire, melee attacks, ticks of DoT damage, everything.
On my other characters, it plays infrequently and when it does, it isn't nearly as loud. Yet on this one character it plays so loudly it drowns out everything else. Only thing I can do with it ingame is to turn off Sound FX in the options, but that really kills the experience in general. Anyone have ideas as to what might cause this or where to start troubleshooting on my end? None of my settings have been changed so I don't think it was something I did. At least not on purpose...
I've had the same issue - a roaring noise that plays constantly during some fights, especially when there's DoT flying around.
I did what someone in another thread recommended for sound issues in general, which is to rename sound.hogg to sound.hogg.old and let the launcher download it again. I haven't tried playing again yet - I'll do that later this evening, and let you know if the issue seems to have gone away - but I've noticed immediately that the just-downloaded sound.hogg is only 637 MB compared to the 713 MB of what I had before. So something happened there, probably during patching.
EDIT: Well, it didn't help, and some interesting things turned up in the process of finding that out. The main one is that the sound.hogg file has grown since I downloaded it earlier this evening! It was up to 660 megs or so when I deleted it. I also deleted the prepatch directory, just to be sure. I'm downloading another copy via the launcher right now and I'm going to make a backup of it in my directory, just so I don't have to do that again with every experimental fix.
The sound itself is a loud growl or roar that sounds vaguely like someone saying "LLLAG" (when combined with a DoT, it overlaps/repeats), with an oscillating hum or warble in the background. Very annoying.
Without more details, it will be impossible to diagnose the cause. Have you attempted logging your combat data and correlating the time the sound occurs with the incoming damage shown in the log? This may help you narrow down the source of the FX and would be useful if you end up filing a bug report for the issue.

I'll try doing this tomorrow and see what I can find. I've almost fully stopped playing this character since it is so headache inducing. My other characters are still fine and this doesn't happen with them, haven't even heard this particular sound effect play on any of them recently either.
As a possible starting place, a quick look at the combat log last time suggested that it might be connected to my team members taking damage to their personal shields - particularly plasma and/or disruptor fire.
I've tried several missions and it didn't matter what kind of damage was taking, this roaring audio cue played every time my character took any sort of damage. Didn't play when my bridge officers took damage, just my player character. I've taken fire from phaser, tetryon, plasma, polaron and environmental damage; as well as a variety of DoT damages. Plays for every single tick of damage. Checking the combat log didn't reveal much, just that I took damage from various sources in just about every flavor available. I've tried swapping kits and shields around to see if it might be tied to abilities or shield procs, but no luck.
Nope - I've seen (heard) it on a Human and more recently on a Joined Trill, both Fed.

.... but my Trill's doctor is a Tellarite. Oy. I'll try leaving him on the ship and see what happens.
I'll also double-check on the Human, to see if it's really happening or my memory has failed.
Update: Left my Tellarite BO on the ship and no constant annoying sound. I suppose I could put him back on the away team, just to be absolutely sure, but... I think the Devs have enough data now, and I'll simply wait for them to fix the problem.

Would that matter in this case? The issue only happens on one of my six characters. The other five don't have this happen. I'll give it a shot though.
Edit: Nope, didn't help any.

HF_Mudd, is your character also a Tellarite?
