Quest - Advanced Grinding Machine

Ever since Jolly Jones quests have been introduced, there is an intermittent problem with the Advanced Grinding Machine quest cruising between servers. Basically, this quest should give the "Perfect Flesh" item from collecting at the machine but about every 1-2 weeks it changes on consecutive servers to give "Rough Flesh" item only instead making the quest uncompleteable. This problem seems to cruise through servers in a sort of a circle - e.g. if it gets fixed on one server, it gets broken on another one, gets fixed there and appears again on yet another, etc. This week it got broken on RT yet again, e.g.
