Gathering items Quest

Hi guys can anyone help me with this quest ( Gathering Items ) says to collect 10 savage fragments and 1 witch relic i keep getting sent from one world to another and back again which is very confusing so any help would be great

sounds like going to the wrong map you need get to Valley of the Scarred which need to get to Old Heavens Tear first which need Chrono Key from Wraiths ploy quest line once got key go messenger of time choose the travel back quest0
