Dragon Quest Error help

I did all the mini quests leading up to the final boss of the level 41 dragon quest. I killed the boss and went to the npc i am supposed to go to to submit the quest. When i get there, i cannot submit the quest. It keeps saying
"Mission already taken! unable to complete quest!"
What happened? does this mean i did the whole dragon quest for nothing? What should i do?

    Look in your quest list for a blank entry next to a bullet point, and trash it (the blank entry). I had the same problem, and that fixed it.
