Its'actually broken i think with the last mod update, mani people after may 2022 reporting this issue.

Master of Madness, for getting gold on the madness variant of Prophecy of Madness, is also broken. Nobody on Trueachievements has recently won it.
It's impossible to get gold on round 1, even with an all-DPS, perfectly geared team camping spawns and instantly killing everything. Someone on Trueachievements went as far as having a full team slot the stronghold +damage to demons overload enchant, Zariel-capable people only, still silver.
Round 2 always ends prematurely, about half way from silver to gold.
Round 3 you can gold.
Combined, a gold rating can't be earned.
