Finished Master Svarborg with no deaths, but didn't get achievement or banner.
DId you beat MSVA with no deaths on your first attempt? If not, were you still in the MSVA instance when the queue for MSVA popped ? If so, that might explain why. In Mod 15, my Guild and Alliance did some MSVA banner attempts. We got it after 3 or 4 attempts. One player did not leave the instance, between attempts, and did not receive the Banner when we beat it with no deaths on the very next attempt. Everyone else, who exited the instance but did not already have the Banner, received it and unlocked the achievement.
