TR Stealth periodically not working (PVE)

Hiya

For some reason, monsters will occasionally attack me with regular attacks even though I'm in stealth. I don't mean AOE or dodgable telegraphed heavy attacks. They will quite literally just run into me and hit me over the head with a normal attack, despite me still running around in stealth. That can't be intended, right?

It kind of undermines the benefit of using a saboteur build... It sabotages the path, if you will.

    Specific monster types are immune to stealth. Not sure what you're fighting to comment further.

    A couple of the monsters in the new areas and a number of new and old bosses can see through stealth and some monsters will make a reactionary attack if hit from stealth. Which monsters and which areas are problematic?

    It's been particularly bad with the Abominal Yeti on top of the wall in Bryn Shander. But from the looks of it, it seems like it's likely that it simply sees through stealth.
