Dread Ring: Rewards from Arcane Gauntlet not dropping to inventory

The Problem:
I'm finding most items collected from the arcane gauntlet coffers at the end of the Dread Ring lair quests are not dropping to inventory. I first noticed this a few weeks ago. I can confirm the problem occurs for multiple characters on my account. I cannot confirm if others' accounts are affected.
Steps:
1) Complete any of the three Dread Ring lair quests while in possession of an arcane gauntlet.
2) Open the Arcane Gauntlet Coffer before exiting the lair.
3) Read the reward screen that pops up with the RNG generated reward.
4) Acknowledge the reward to close the pop-up screen.
5) Open my inventory to check for the rewards.
Observed Result:
In many (but not all) cases the reward that was shown is not in my inventory.
Expected Result:
The item should be in my inventory.
Items I've Checked (so far):
MoP Rank 3 - Does not drop (observed at least 2x)
MoP Rank 4 - Does not drop (observed at least 2x)
MoP Rank 5 - Drops to inventory (observed 1x)
Shard of Vorpal - Does not drop (observed at least 1x)
Sapphire - Drops to inventory (observed 2x)
Emerald - Drops to inventory (observed 1x)
Other notes:
1) I have room in my inventory at the time and there is nothing in an overflow bag
2) I have checked for stacked items and in such cases quantities do not increase
