TR- Blitz not proccing all the time

Obviously running TR, I've only been in this game for around a month, but according to Xbox i have 144 hours into it. Not a lot compared to other people and time i have in other games I've played, but enough to notice a change.
After the update two weeks ago i noticed my TR Blitz wasn't proccing all the time. My setup for encounters has me go invis hit Blitz and wipe mobs before they can react. After the update i found that maybe 5-7% of the time my char goes through the motions, but no damage is dealt (no hit markers, no hit SFX) so a i get swarmed.
This happens on all maps. I'm on level with mobs, so my blades don't go above or below them. I've only been able to notice this with blitz. I was hoping it would have been fixed this week, but it hasn't.
For other reasons i have done these things:
1. Log out, log in
2. Clear Xbox cache
3. Reset temp mac address
4. Uninstall and reinstall NW
