Companions staying dead

In T9G at the end fight with Ras Nsi if your companion falls off during his hand sweep then it will stay dead and can't re summon it. This never used to happen and it can ruin your run if multiple people lose their companions. Im noticing my companion is unsummoned way more than it should be after running dungeons.
