Anyone else have their ps4 rev like a jet engine when they play neverwinter? It happens more often than not, and lasts for quite a while before calming down.

Yes, but it also does it when I play Horizon: Zero Dawn and No Man's Sky. I have a tray fan for it and it will kick in, cool it down and it goes back to normal for a while. *shrugs*
You do know that it is your Hard drive that is causing that noise. If you want a quiet PS4 you have to upgrade to a SSD. Upgrading to a Hybrid drive I noticed my Hard drive does not run as long as the standard PS4 hard drive.
As for performance gains, a hybrid offers the best of both worlds. It is faster than a standard hard drive and provides larger storage at a lower cost.
If cost is not a concern, upgrade to a SSD for faster and quieter system.
