Armour/weapon enchantments since Fangbreaker

I've been having certain problems with the game for months. Ever since completing the the Fangbreaker Island quest (not the dungeon; the Cold Run quest where you earn Makos' Signet Ring) I have had the following issues:
- My armour enchantment (lesser soulforged) no longer functions. Removing and re-equipping the armour and removing and re-inserting the enchantment does not fix it.
- My weapon enchantment (lesser terror) has no effect either.
- Every time I accept the daily "complete 3 quests" quest in Bryn Shander, Lonelywood or Cold Run, or enter any of those areas while the quest is active, I get the message "You can only have one of a unique item" without and indication of what the offending item is.
I'd be grateful for any insight
