The Dullahan.
Supernatural hunter of Celtic myth, this pursuer of the dead and damned is interesting indeed.
Owing no true fealty to Heaven or Hell, but following a code of vengeful justice known only to the Dullahan itself, they exist to reclaim the dark souls who escape their eternal punishment and to deliver judgment upon those whose lives of unrepentant evil guarantee them that such tortures are due when they do finally cease to draw breath. Indeed, some even claim the Dullahan's own weapon is forged of iron leeched from the burning blood of its victims, and its armor reinforced by the sin-scarred bones of their corpses... though, obviously, none can say for sure if such extreme claims bear truth in them.
Just as Iron is harm to the Fae, and Silver a bane to Were-beasts, likewise also is pure Gold anathema to the Dullahan, its symbolic reflection of the blessed Sun a shun against the Dullahan's unhallow'd power - it is not the "holy ground" that causes one to avoid churches, but the golden icons of saints that are in it. But nothing can repel them permanently, as they are inevitable in their pursuit, with locked gates and barred doors opening of their own accord rather than hinder the Dullahan's pursuit of justice upon the damned.
While the common depiction of them is a headless being with some manner of smoke or fog emerging from the stump, in truth the Dullahan does possess a head, often kept hidden away lest it be stolen and the bond to it used for dark deeds against them. Should it so choose, the Dullahan can even take on a guise of mortality by reattaching its head, with only a neck-encircling scar (usually masked by a scarf or collar) to give it away, with its mortal guise even causing it to suffer the sensations of fatigue, thirst, and hunger just as mortals do, though they remain immortal and the guise can be shed by removing their heads once more.

Figuring out a good way to manage the "headless" effect is the hardest part... I've seen one or two "headless" costumes around the game, but never got anyone to explain how it was done. That, and I've heard there were some ways that got hotfixed, so are useless to try now anyway...

I've also recently fitted her with an Elder Worm Humidity Dome and a ton of Recovery, so she's also all ethereal and ghost-like most of the time.

Select Huge Rigid head and choose one of the options that show an empty headpiece. In my case I have over a dozen, but it depends on your unlocks. For a hooded version, the archer hood works like the Nighthawk one did.

You can still use the tailor, but you cannot amend the costume with the hood as it removes it automatically. However, if you accidentally load the wrong one, just hit back and reselect and it will be untouched.
Can you give the source of this story? There are details here I've never seen before.
It's not from a singular version of the myth, but a personal take on trying to make a unified form of the being, with the details cobbled together from multiple variants I've heard over the years.

That, and my computer that I play CO with hasn't been able to get online for a couple months - I'm borrowing a family member's PC to write this.

Figure a way to pull that off without the mask, and it'd be perfect.
Most of them either were from the C store or from lockboxes. However, try the AH, you might find a piece there. Amongst the pieces you can use the Alien Gladiator helms, 2 and 3, the Bruce from Victorian set, the sugegasa hat from the Robot Samurai set and the kabuki, head spikes, plague mask, dinosaur skull and head cage from the Halloween mask pack. The feather mask also works, but is less obvious as are the dryad and dinosaur suit heads.

What would be ideal would be a way to do it so it had a "cover" for the neck-hole like the Floating Brain thing, but minus the bubble and brain. Then just use an aura or something to emulate the smoke or whatever.

One of the coolest adaptations of the Dullahan concept was a (somewhat NFSW in places for transformation and nudity/sex moments) story by an author on DeviantArt by the handle of psudodrake that blended it with a few other legends like the death-hound Barghest and such and tied it all in with the Sleepy Hollow legend and even gave nods to Ghost Rider... and the main character was a woman.
The holoforce set makes this more than possible: Go to your head category, change the classification to "huge helmets rigid", and change your head type to "none" and helmet type to "holoforce". Color that helmet black, and switch the material to either Holo-hex or Psionic.

Cool... now, what aura would work best for this? I know some auras can "re-visible" the holoforce pieces (I use the trick and the Id Mastery aura to make "psychic armor" for my Disciple character) but in this case, I DON'T want the stuff to reappear...
Any store-bought head aura. Especially considering how expressive and specific a fire head aura or vibration head aura can be, these work best. Eyes, also.

Shadow Form - pros: makes a very large black cloud appear for your head, as well as glowing eyes. Cons: cloud appear all over the rest of your body and turns primary colors on all your costume pieces pitch black.
Any store-bought head aura. Especially considering how expressive and specific a fire head aura or vibration head aura can be, these work best. Eyes, also.

Cosmic Knight was made just for Dullahans like this. It includes various 'cosmic flame' auras built into costume parts, which can be applied to the headless option to give yourself a very distinctive, constant head glow.
Also good in that it won't cause a holo-helmet to suddenly appear whenever you use Conviction!

Oh, then that is PERFECT. My two biggest anxieties about this concept were the sudden helmeting and having to bother buying an aura-pack just for using one part on this character - so glad to hear the set has workarounds for that.
