Okay, seems STO players have been having the exact same problems recently. This is not just a problem with CO. But it also isn't necessarily a problem with Cryptic, at all.
It seems the culprit is Cogent, widely known for having really uneven (ie,
crappy) service from one region to the next. This actually explains a lot. After examining various traceroutes and comparing them with what others from the DFW area are experiencing, the specific hop we are having trouble with is te9-4.ccr01.dfw03.atlas.cogentco.com, which is my first hop into cogent from TWC.
Not everyone who has the problem lives in DFW, of course. Or even in North America. What we all
do have in common, however, is that everyone has to go through cogent to reach Cryptics' game servers.
The only thing you can really do is run a traceroute app (one that allows you to poll all hops continuously at a HIGH rate), identify the high latecy/high packetloss hop, and then call your ISP with the details. Someone over on the STO forums posted a really good bit of advice, which I'll repost here:

I won't bother actually posting the results of my traceroutes unless someone on here reaaally wants me to. Because I'm lazy :P But polling the bad hop returned a 220+ ping and 1-3% packetloss - consistenty - whenever problems ingame popped up. The only thing you can blame Cryptic for is going with a provider who has a rep for terribly inconsistent service.
So...there you have it. All you can really do is start calling, and hope that someone at your ISP is willing to actually do their job and badger someone else at Cogent into doing theirs. Good luck.

Don't just post here. Post anywhere on Perfect World that seems appropriate. Let them know we aren't happy about these connectivity problems, and if they can't pressure Cogent into providing reliable service to them, then PWE/Cryptic needs to find a different provider.
Good Job hitting it right on the head, I hope they decide to fix it, but yet again I'm going to play the skeptical bad-guy here and say we aren't worth their time.
I mean, I really want them to look into it and fix the problem, but knowing their track record for fixing things that are broke I won't hold my breath. I mean just look at how buggy and broken so many aspects of the game is.
I pretty much just see myself getting no support until my 3 Months are up and just unsubbing.
I don't want to, but they will likely force me to, as will they you.
EDIT: Forgot to add that I did a tracert and it is indeed a Cogent hop that breaks it.
It's been a problem before, but it's been a while. I thought maybe they had finally gotten their act together...

Thank you for getting to the bottom of the issue. You got a response from someone that may or may not be official (?) over on the PerfectWorld thread.

Cogent is not likely to ever get their act together. "Bare minimum" is basically their corporate strategy.
They'll get their act together if Perfect World threatens to take all that juicy activity away from them by switching to a different ISP hub.

Same here. It's not Cogent. My route doesn't even touch it.
Your problem is not my problem.
You two are having the constant Disconnect issue, and my issue is a different one, I now understand that.
I have a network lag issue, not Disconnecting.
Even if I set all my graphics settings on super low and turn on half resolution, I.E. Playstation 1 mode, I have terrible lag and FPS due to poor network connection and lost packets.
And it is Cogent.
