Just do Lightning Reflexes as Slotted Passive and CON as PSS or SSS with a lot of Heroic Gear statted with CON. Then pick Tank role and Bulwark from Protector tree spec. You can also do Way of the Warrior with Heroic Dodge/Avoid stats and Parry Elusive Monk might also be needed.
Of course the issue is that your other SS have like next to nothing. In some builds, that is perfectly acceptable. I don't think this will work with Power Armor since you need a lot of energy.
I'm personally not a big fan of LR these days, since similar levels of dodge/avoidance are fairly easy to acquire with powers, specs and gear.
To the op; you seem to be asking for an invulnerable/dodge build. Grab Dex and Con as superstat, with an energy superstat as a third depending on your energy unlock.
You need Dex for Parry w/ Elusive Monk and crit chance. Keep in mind that Elusive Monk only procs in Melee. Then grab invulnerable for your passive, and you have the basic setup. Stack dodge/avoidance with gear/specs/powers. As a tank, advantaged thundering kicks or advantaged dragon kick are nice. For ranged, the dex spec tree is helpful, with evasive maneuvers a good choice. Remember that Sleight of Mind is not helpful if you want to hold aggro!
To add to resistance you could consider Mighty Kick /w Madness?! For Melee or Breakaway Shot for ranged to get a stack of Defiance. With Con as one of your superstats this will add 10-14% on top of your invulnerable.
For Spec trees consider Guardian and/or Warden for the Fortified Gear/Best Defense combo, or both for even more survivability. As stated by Monsterdaddy, Protector's Bulwark is a very good choice for a tank, since the hybrid role allows you to do a lot more damage than the Tank role. Even though Bulwark's threat caps lower than Tank innate threat bonus, the extra threat generated by the extra damage of hybrid more than makes up for that though.

You can take any passive really. LR + a couple dodge boosting powers, or anything else plus a bunch more dodge boosting powers (Evasive + Thundering Kicks both R3 adds 42% dodge for 8-15 seconds). Anything that adds dodge rating hits diminishing returns fast with LR, but adds a fair bit if you have a different passive. So either way you want powers with flat % adders for max effect.
