Nemesis despawn bug

My nemesis will despawn seemingly at random after being engaged. I've left and re-entered mission, dropped mission and reacquired, edited the nemesis... out of ideas.

My problem with nemesis system is when the minions appear they disappear before I start swing, what's the problem.
