Vapor Form bugged?

Each time I use Vapor Form, all abilities get removed from my action bar. When I switch back to normal form, only some of my abilities come back. This means I have to open my abilities window and drag them on after each use, effectively breaking combat.

    I can't replicate this bug. Is it always certain abilities or certain positions on the hotbar that are not restoring after the effect ends?

    I can only replicate it inconsistently, but it's usually the first two slots, default keybinds 1 and 2 that don't restore. Once it was that entire bar, another time just one slot. It feels almost completely random.
