Dazee covered most of the basics of it. If ya want more reading on 100% dodge builds, ya can always just do a search for them on these subforums- Monsterdaddy and Selphea have threads on them.
Else, I'll reiterate the point not to seek out extra Avoidance rating if ur using R3 LR. Avoidance's DR (for both what's labeled as rating and as '%'- the latter is not actually a flat percent cause it shares the DR) is very harsh, and getting avoidance well past ~78-80% isn't really worthwhile for the stat investment it'd take.
In general though:
- CON PSS / deflection
- R3 Thundering Kicks and Crashing Wave Kick w/ adv
- Legion's BP of Elusiveness + R7 Gambler's or above
- As much DEX as you can afford.
DEX primary / LR / thundering kicks / crashing wave kick / parry can work, but not easily.
I have a build that does uses those to get to 100% dodge chance, but that requires about 750 DEX.
Note that you can also just build around less than 100% dodge, and being tough enough that the occasional missed dodge doesn't matter. A Str/Con Warden/Vindicator build using Laser Knight can still maintain upper 80s dodge on top of 15k hp, and will take 7-8k from a failed dodge against Frosticus.
80% dodge is already exceptionally good. On my Dex-based build I already find myself performing pretty well in survivability with R3 LR with 70% dodge with R5 - 6 Dex mods on a mix of 2 Heroic and 1 Legion gear. The only other thing I rely on to boost my dodge is a 42 second recharge timed MD that cycles pretty often during combat (thanks to the Heroic helm).
Be prepared to invest in a full set of Legion's gear and as mentioned, R9 Dex mods to get your Dex as high as possible to overcome the heavy diminishing returns if you insist on a perfect 100%.
This is true. I have tanked gravitar on one occasion with a Specialist AT with a 68% base dodge chance, and 5.5k HP.
I had to pay attention and try hard not to die, but I survived, and kept her aggro till the end, even as people gave up thinking it was a lost cause, by the time she went down, we had only 3 people in the rampage.
Yeah, similarly the LR build I mentioned in my previous post is one of only two "tanky" builds that I have to confidently go toe-to-toe with Gravitar.
Consistent 100% dodge was never necessary, apart from 15s of MDs in-between 20+s intervals and some personal healing backup like BCR+RR.
I'm already hitting 100% dodge with that silver champion recognition dodge gear , hoping the RNG is nice to me so I can get them shiny Justice gears.
Incidentally, Swinging with Flipping will also get you a few percent dodge rating (I believe it runs into the same DR as dodge rating from gear, it's not a flat add, but the tooltip isn't very specific).

Yeah its not flat- Swinging's adv is on the DR too. The tooltip is indeed misleading for it.

It's definitely not the easiest thing to maintain the dodge.
