'Stealth' not working so well...

Stealth seems borked to me 
I find it only functional for initial attack, as it doesn't seem to want to reactivate, even when I use a threat-wipe on enemies.
The time it takes Stealth to reactivate feels like an eternity; and that's compounded with Stealth seeming to be less effective now, as I can barely sneak up on anyone anymore.
