Um, I've been away for a year or so and wondering has the game difficulty been increased? I started over from scratch and I swear that the missions weren't as difficult back then as they are now.
One year? Not quite. Closer to 18 months since certain factions (Demon, Worms, Kings) went from pushovers to "think about who you stomp first" (ie: the first Nightmare Invasion). Certain styles/builds/ATs also have more challenge with the early game than others.
Absolutely silly question: did you open up your starter gift boxes and put on your gear? I've forgotten that in the past, and it definitely makes a difference.

Which missions you talkin' about? Maybe you've just been playing those other easy games and forgot how hardcore this souls-like is.
As said the enemy factions DEMON, VIPER, Elder Worms, and Trey Kings have gotten a little tougher and have learned some new abilities...
so sarcasm about dark souls aside yeah some parts might be slightly harder, early missions vs the gangs in westside haven't really been changed though.
There are some parts of CO where it really IS stupid hard and players who suck will get creamed... repeatedly. Try wandering around the Vibora graveyard if you want a challenge. None of the individual enemies is all that hard, but there are loads of them.

If you start from scratch without a subscription or with the free basic gear then it does feel a little tougher than it did. Some general enemies have been buffed but mostly you'd have to be level 35+ to notice. Open Missions and Event enemies are tougher than they were, though, and - currently - some of the Nighthawk one don't spawn at the level you'd expect (they're marked as skull level, but sometimes they spawn as the level of the person who attacked them last time!).
the skull thing is mostly just their HP. They have a set HP pool, that might have been set because a level 40 was flying past at the wrong time. But their attack and defense scale properly.
