If you're sold on a self-rez power, I tend to prefer Thundering Return or Aurora over Rebirth for a DPS character. I like Rebirth on support characters, since they can easily heal through Rebirth's self-damage. But on a character with limited self-healing, Rebirth's damage ticks can put you back in peril if there aren't any mobs to blast. Resurgence can remedy that, but I'd rather pop Resurgence prior to the initial death.
With Concentration as the toggle, I assume you're gearing mostly for your EGO superstat. I'd recommend switching the point allotments in Follow Through and Sixth Sense (3 points in Follow Through; 2 in Sixth Sense), to get the most out of their respective scaling.
Lead Tempest's Tread Softly is handy if you're gearing for dodge/avoidance. But if you're not geared that way, you might prefer the extra damage of Rank 3.
