Update 2.1 Added several encounters to the Ritual place in the sewers. After the summoning event, some undead spawn to keep with the fact that Niahmas causes the dead to rise in his presence. So the run back out has a purpose.
Update to version 1.7 Addressed many of the problems spotted by Melinden above (Thank you) also added a ton of detail to the ritual map, went back and split several of the large blocks of text into smaller paragraphs so it's easier to read. Update Repairs from the Tiamat expansion, You will no longer get stuck in the ground of the Chaos Inn (Chapter 3) or the docks leading to Cutthroat Isle. The inn's cellar is now enterable before you enter the Inn.

Update Repaired this one due to the Tiamat update bugs, I had to change some things and repair a few of the traps and encounters. In the process, I changed several of the encounters. Version 3.5
Update: Fixed the remaining NPC's on the Strongholds "Hard Mode" so that the additional encounters won't attack them, added more detail to the barn area and the Kobold pens.
