PVP Heroic encounters cannot be finished. Stopping at 90%!

PVP Heroic encounters are buggy and cannot be finished.
Which makes the task "make 50 PVP HE" very difficult to achieve.
Problem 1.
More common in DV.
The possibility of mining stops at 90-95%. No more Black Ice chunks are spawning. The HE cannot be finished.
The opposite fraction can summon the Juggernaut which appears fine but the HE cannot be finished this way neither!
There is a slight possibility to finish the HE. If noone from the other fraction is starting to capture points or interfering the HE can be achieved.
Problem 2
More common in IWP.
The Black Ice domination is not even starting. The Black Ice Domination icon is appearing in the map but you cannot start to capture points even if you are standing by the points.

As to domination not even starting, I have nothing for that one.
