This has most likely been asked before and I might be sounding derp but when you run instances do you need a dedicated tank/healer to run them?

Apparently no, possibly for the high end stuff. There isn't a true healer, as a cleric I am mostly support throwing out those bonus heals on top of potions while also doing very decent damage

No, people are running level 60 instances without clerics. Few videos posted earlier. It's not that clerics are useless and people would choose to leave them out, they just aren't necessary, like TRs and CWs. Probably the same for defenders, I just haven't seen videos yet.

It's D&D. The Trinity system is usually left at the door. This isn't to say that you can't run it if you don't want to, though

Cleric can actually do quite a good job of healing and supporting when played right. It just means using the Daily powers correctly, using the Tab skill correctly, and slotting the right skills.

So far as grouping up to the 40's as a cleric I've been complimented in groups for the heals and support that I provide. It really just comes down to your play style, your group, and what you want to do. It's nice to play without the holy trinity but still feel like you have ROLES (cough Guild Wars 2 fail cough)

Just as a by the by I recommend checking out this post regarding the 4 roles of Leader, Defender, Controller, Striker rather than the Trinity idea used in other games. Also contains a wealth of other D&D info. http://nw-forum.perfectworld.com/showthread.php?23911-Brief-Lore-Intro&p=370031&viewfull=1#post370031
