Thought I would post this in here since it seems relevant.
After queueing for a dungeon and entering I would say 90%+ of the time you load into an already cleared instance. There needs to be a way to either prevent this from happening or to reset the dungeon. Some people have stated you need to preform a group and run down to the actual entrance and it will work. I have not had a chance to try yet since its late, I will tomorrow though.
I also tried putting in a ticket about this however it feels like the ticket never submitted even after putting it in.

Manually forming the group and meeting at entrance works. No luck for me otherwise.
I don't think you need to run down to the entrance, although we did do that. Pressing F on the entrance to a dungeon only puts the party in queue apparently, you don't get a "gather your party" prompt like you do for instances. So just having a full group before you queue (K) should be enough. 'LFM' refuses to die .

I also encountered this issue and have submitted a ticket. Though I don't think there is a way to track your ticket. Would be good if there was an acknowledgement that your ticket was successfully submitted, or the ability to see a list of your submitted bug reports. With the option to update them.
The Clock Tower Had This issue. Join a Group and all we had was No Mobs, Nice place if you walk though it since no mobs. This issues needs Fix Baddly

The only time the Cloak Tower instance would work was if you got a full team, and *then* queued. I also noticed that it said you were leaving a PvP Instance when you left it, even though it wasn't pvp. Not sure if that was part of the issue.
