Please look at the aggro radius of the Remorhaz in Cold Run. They will constantly fireball you from very far distances. Something changed because it used to not be like that.. thanks.
I bug reported this last year just after mod 16 launch on PC but it was never acknowledged.
To add some detail, the mobs attack range exceeds the range where you are even considered in combat with them. A hit will put you into combat, but only for a second or two. In order to complete the intro quests for Cold Run, a player now has to kill every Remorhaz in the zone to be able to activate the scrying stones. Most likely, they will be respawning before you complete the circuit, especially on melee characters. Especially fun for melees when they are getting the big AOE that prones from 2 or more mobs while they try to fight one.
This is by far, the most obnoxious of the m16 'featured' mob overhaul.
