What steps have you tried to resolve this? Where in the world are you located. Who is your ISP and what kind of Internet are you using?

I normally can play with bit of lag, but still playable. Only in huge groups like Tiamat it's become unplayable again.

OK, you have a weird routing - you start internally, then you have a private wide address (probably your ISP?)
Somehow you leave Europe and go to China? (218.30.39.69 - China) - all very low pings.
Then you hit another server in China, where your pings go up by an avgerage of 120ms, THEN you finally get to the states. You will note that you bounce around cogentco servers a lot, and you are losing pings during this - you have not even reached the game servers and you are already losing pings just on a tracert. Try this:
    ping 154.54.24.233 -t
It will probably show you dropping large numbers of packets at that point in your journey - nothing to do with the game, but all to do with your provider and who they use in the States... During peak times, it will be bad, at other times you are probably OK.

My guess is that as you are routing through China, your getting extra "attention" by the States (or China - I bash equally here :) )- can you try another ISP provider, maybe via a prepaid wireless?

Might is not always right - the powerful sometimes forget that.

BTW, do NOT open your router ports! Close them immediately - unless you want the world to try and hack into your computer, that is. Unless you are running a server (or similar) for other purposes, it will not help with Neverwinter at all - it only sends information back via ports as requested by your system, not just "out of the blue", which is what port forwarding is for - any request, from any system to YOUR port on your system...

Thank you very much. Helped me alot to understand what is happening. But all ISP provider here is not good at all... =/
Have any chance to be my modem/router? I had problems with him before...

It doesn't look like it's your modem. It looks like it's your ISP making you go through weird routing.

I second what @kreatyve said - highly unlikely your modem/router. You could try using a VPN service like
    PingZapper : pingzapper.com/, or
    WTFast: https://wtfast.com/

But do not pay any money until you have tried it and it works for you.
