Groups forming with less than 5?

This is happening frequently, the queue pops and I accept, get dropped into a group with anywhere from 1 - 3 other players. Does the system fill in the remaining members or are you supposed to disband and requeue?

This is extremely annoying given the already long queue times.

I have seen this sometimes. Usually in CN queue.

My theory is that a fail group happens, then people leave and requeue which gets them the same group, then they start declining the group which ends up in you a new person queuing hitting the 3/5 or 2/5 party.
