This still is a big issue in the foundry you set an objective and place it on your map a couple of saves later, every time you play through the map that asset show up off the map. I first bugged this during the foundry beta. This makes the foundry useless for a lot of us. There are many published quest out there that have the same issue, and folks can't comment on them, because if you don't complete the quest you can't give feedback.
This really needs to be a priority to fix before the game actually launches, and it is yet another reason I say the Foundry is still in a alpha state and is not ready for prime time.

A couple of things that might cause items to appear off the map. Placing an object on a multi-level map, you may have to adjust the Y value to get the item on the plane needed.
Also, if you place a room on the map, rotate, then populate, the items will appear off the map. Place a room, populate it first before rotating.
Other than that, I haven't run into your scenario yet.

Type /locvec while standing where you want to know your location.
