I was in a tiamat run, after the first heads phase (4 heads down only), the instance never geared into the clerics protection phase and the timer was stuck at 25 mins.
We encountered similar issue yesterday - we burned Tiamat down in one smooth swipe, but the heads just stayed there hanging afterwards - with the timer stuck at 25 mins. (don't know if forever, after few mins we disbanded and everyone left the instance with no reward)
Haven't had this bug happen to me, but the timer has been broken since the release of Mod 13. It no longer counts down and you're therefore stuck there indefinitely unless you can get a "vote to abandon instance" pushed through (not sure how long the cool down on this is in Tiamat) or take a leaver penalty.
Logic is the beginning of wisdom, not the end of it.
