Potion Tray Items Not Showing

This is only on my DC. The items in my left and right D-pad spaces aren't showing. All I see is a red, like it is not useable, covered basic potion vial. No matter what I put in those slots. It does show the number of the items that I have and let's me use them but I have to remind what I have equipped. My other 7 characters do not have this problem. It's it because I have healing potions there on a DC?

    Just put my invoking blessings in the tray and the icon remained a redded out potion vial. It is the same image if the tray is empty too.

    The bottom slot shows correctly.
