epic demogorgon achievement bugged?

ran 2 successful epic demongorgon runs. all were gold at all 3 stages, yet at the end when the final score board appeared it shows only 1 phaze as gold other two phazes (2+3) do not appear at all.

so i would like to know is it a bug? does this effect loot and loot from chest?
