In and out Combat mode every second

Anyone else having this issue. i mean its not limited to any map zone or quest its just go in and out of combat mode ever second for a min after combat unless any other monster is even kinda close then it just keeps going forever.

    It is more than likely either a power you are using, some armor piece you have or something similar that is trying to deflect. They tried to remove all of them, but with all the changes in Mod16 maybe something was changed back, or missed.
