What are the causes of InvalidNameEntity?

I've made some bad assumptions that the result of InvalidNameEntity while inspecting in a guild roster means that specific character has been deleted or the player deleted their account; however, I am apparently horribly mistaken. Can anyone from ARC confirm the cause or causes for InvalidNameEntity to appear? If it doesn't indicate a character or account has been deleted, is that indicated a different way? Thanks for your time.

General consensus (Cryptic will not comment) is that it seems to indicate a character that has not logged into the game for 30 days, although some speculation is that it can happen after 14.
